#1 - (Online First) Septic osteoarthritis caused by Bacillus pumilus in a green iguana (Iguana iguana)

Abstract in English:

A green iguana (Iguana iguana) was presented with acute swelling of the left knee and reluctance to flex the joint. Diagnostic imaging findings were consistent with septic osteoarthritis. They included progressive osteolysis and osteoproliferative changes affecting the distal femoral and proximal tibial epiphyses, as well as intra-articular effusion, which were monitored using radiography and ultrasonography. Bacteriological culture of synovial fluid, followed by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS), identified Bacillus pumilus, a saprophytic bacterium rarely associated with disease. Despite antimicrobial therapy guided by susceptibility testing, the osteoarticular lesions progressed, ultimately requiring limb amputation. Histopathological examination confirmed chronic granulomatous osteomyelitis. The patient recovered uneventfully, with no recurrence of lesions during a 1-year follow-up. Serial imaging was essential for monitoring disease progression, refining differential diagnoses, and guiding surgical decision-making. To the best of the authors' knowledge, this is the first report of osteoarticular infection caused by B. pumilus in a reptile, highlighting the potential role of opportunistic environmental pathogens in the pathogenesis of musculoskeletal disease in lizards.

Uma iguana-verde (Iguana iguana) apresentou edema agudo no joelho esquerdo e relutância em flexionar a articulação. Achados de imagens foram consistentes com osteoartrite séptica, incluindo alterações osteolíticas e osteoproliferativas progressivas na epífise femoral distal e tibial proximal, bem como efusão intra-articular, as quais foram acompanhadas radiografica e ultrassonograficamente. A cultura bacteriológica do líquido sinovial, seguida de espectrometria de massas por ionização/dessorção a laser assistida por matriz com analisador de tempo de voo (MALDI-TOF MS), identificou Bacillus pumilus, uma bactéria saprofítica raramente associada à doença. Apesar da terapia antimicrobiana guiada por testes de sensibilidade, observou-se progressão das lesões osteoarticulares, e procedeu-se à amputação do membro. O exame histopatológico confirmou osteomielite granulomatosa crônica. O paciente recuperou-se sem complicações no pós-operatório, sem recidiva das lesões após um ano de acompanhamento. Exames de imagem seriados foram fundamentais para avaliar a progressão das lesões e refinar os diagnósticos diferenciais, auxiliando na decisão cirúrgica. Até o presente momento, não há relatos na literatura de infecção osteoarticular causada por B. pumilus em répteis, destacando o potencial papel de patógenos ambientais oportunistas na patogenia de doenças musculoesqueléticas em lagartos.
