#1 - Accuracy of ARFI elastography in the differentiation of cataract stages in dogs

Abstract in English:

Objective was to evaluate the accuracy of elastography in the differentiation between normal and cataract lenses One hundred forty-five eyes of 98 dogs were divided into groups according to cataract stage. Forty-twoeyes were submitted to phacoemulsification. Biometric parameters, echogenicity and echotexture patterns of the anterior, posterior and vitreous chambers, lens and retina-choroid-sclera complexes were evaluated by ocular ultrasound in modes A and B. Deformability, and color (blue color = indicated less rigid structures, color red = more rigid structures) of the lenses were evaluated by the elastogram. The shear wave velocity (SWV; m/s) was calculated in three regions of the lens, both in the cortex and in the nucleus. The SWV of nucleus was statistically different between the normal lenses and with cataracts, and between the stages of cataract (P<0.001). Healthy lenses and incipient cataracts had a more rigid nucleus. Mature cataracts presented lowest nuclear rigidity (P<0.001). On cortical region the SWV was significantly higher (P<0.01) in intumescent and incipient cataracts. SWV less than 2.67m/s indicates cataract with a sensitivity of 72% and specificity of 94%. Values lower than 2.23m/s suggest mature cataract, with sensitivity of 71% and specificity of 76%. SWV greater than 2.66 m/s are associated with normal lenses or incipient cataract, presenting sensitivity of 94% and specificity of 84%. Qualitative method allowed differentiation between healthy and affected lenses and the classification of evolutionary stages. There was a correlation between the degree of stiffness of lens in cortical and nuclear regions (p=00165, r=0.37) and between the balanced saline solution quantitative and surgical time (P<0.01, r=0.73). Degree of stiffness of lens did not correlate with parameters of phacoemulsification. Elastographic proved feasible for evaluating the lens of dogs, characterizing the types of cataracts, and demonstrating increased stiffness of the diseased lenses.

#2 - Concentrations of matrix metalloproteinase-2 and interleukin-1 β in the aqueous humor of dogs with normal and cataractous eyes

Abstract in English:

We aimed to determine the concentration of MMP-2 and IL-1β in the aqueous humor of dogs with healthy eyes (n=8) and in those with mature (n=8) and hyper mature cataracts (n=8). Correlations between cytokines, cytokines, and intraocular pressure (IOP), as well as cytokines with ages of patients of each group, were also assessed. In patients with cataract, aqueous humor was collected at the end of the electroretinographic procedure. In healthy dogs, aqueous humor was collected before elective surgeries. Cytokine levels were determined using ELISA. IOP was assessed by applanation tonometry. IOP of patients with mature and hyper mature cataracts were lower than the ones measured in normal eyes (P=0.158). MMP-2 aqueous humor concentration was higher in patients with hyper mature cataracts, in comparisons with healthy patients (P=0.04). Average IL-1 β aqueous concentration was higher in patients with cataracts (P<0.0001). Significant higher values of IL-1 β were observed in patients with hyper mature, than in the ones with mature cataracts (P=0.0085). Correlations between MMP-2 and IL-1 β (r=-0.38, P=0.06), MMP-2 and IOP (r=-0.149, P=0.484), and IL-1 β and the ages of patients were not observed (P>0.05). IL-1 β and IOP correlated negatively (r=-0.42, P=0.04). MMP-2 and the ages of patients correlated only in dogs with mature cataracts (r=0.772, P=0.02). It can be concluded that the increment in the aqueous humor concentration of IL-1 β found in dogs with mature and hyper mature cataracts, in addition to the negative relationship of this cytokine with IOP, suggests that IL-1 β is involved in the pathogenesis of LIU. Despite dogs with hypermature cataracts presented significant higher concentrations of MMP-2 in the aqueous humor, such cytokine did not correlate with IL-1 β and IOP. In our study, a relationship between aqueous humor cytokines and the ages of patients was only confirmed between MMP-2 and the ages of dogs with mature cataracts.

[Molecular evaluation of the anterior lens capsule of older dogs suffering from high risk cataracts.] E-mail: ana6113@usp.br Nowadays, the anterior lens capsule and its epithelium have been being frequently studied aiming to reduce the incidence of posterior lens capsule opacity, a complication that frequently occurs after surgical removal of cataracts, due to epithelium cells differentiation and migration to the posterior pole. The objective of this study was to evaluate by histochemistry (PAS and picrosirius red) analysis two important molecular components of the anterior lens capsule (proteoglycans and type IV collagen) in older diabetic and non-diabetic dogs, with diabetic and hypermature cataracts, after phacoemulsification surgery utilizing 0.1% trypan blue or not. Twenty seven dogs, including 21 female and 6 male dogs, with ages varying from 8 to 12 years old (mean = 9.6 yo) of different breeds were studied. The animals were divided into 2 groups: GC (hypermature cataracts) and GCD (diabetic cataracts). Results showed that, besides their different pathophysiologies, both types of capsules studied (diabetic and hypermature ones) presented the same molecular composition of proteoglycans and type IV collagen, since no statistical significant differences were observed. In addition, 0.1% trypan blue was not capable to induce any other evident alteration for the samples. In conclusion, our findings suggest that, if the results consist in alteration induced by the aggressive metabolism of the diabetic cataract or the chronicity of the hypermature one, it is of the same intensity and independent of the use of 0.1% trypan blue. It is also possible to suggest that this alteration must be capable to compromise lens epithelium cell metabolism, which should probably favour future lens posterior capsule studies.

[Molecular evaluation of the anterior lens capsule of older dogs suffering from high risk cataracts.]

#4 - Normal values of b-wave responses of full-field electroretinogram in Lhasa Apso dogs with cataracts according to age, 35(3):274-280

Abstract in English:

ABSTRACT.- Góes A.C.A., Villar T., Otsuki D.A., Lisak R., Pecora R.A. & Safatle A.M.V. 2015. Normal values of b-wave responses of full-field electroretinogram in Lhasa Apso dogs with cataracts according to age. Pesquisa Veterinária Brasileira 35(3):274-280. Clínica Veterinária Vetmasters, Avenida Pacaembu 1166, São Paulo, SP 01234-000, Brazil. E-mail: asafatle@gmail.com Lhasa Apso dogs with immature, mature or hypermature cataracts were divided into four groups according to their age (G1: 1 to 3 years old, G2: 4 to 7 years old, G3: 8 to 11 years old, G4: more than 12 years old). All animals were evaluated under the same sedation protocol to allow the performance of the electroretinogram (ERG) exam to determine normal value of b-wave response of the full-field ERG according to age. Three ERG responses were recorded: rod, maximal and cone responses. The amplitude values and b-wave implicit time of the responses of all groups were compared and analyzed by Kruskal-Wallis test (variance analysis for non-repeated measures), followed by the Dunn post-test (when p<0,05). A significant decrease was observed in maximal responses’ amplitude, when comparing the G4 group with G1 and G2. No statistically relevant differences were observed in the b-wave implicit time values between groups. The ERG values are directly influenced by the animal’s age. Older patients presented a decrease in the amplitude of the maximal response. The study determined the normal parameters of ERG b-waves for Lhasa Apso dogs with cataract according to their age group.

E-mail: mpgalego@uol.com.br Cataracts represent the leading cause of blindness in dogs. The second most common cause of cataract in dogs is a result of metabolic alterations caused by Diabetes mellitus (DM). Ultrasound biomicroscopy (UBM) is a high-frequency (50 MHz) ultrasonographic method that produces B mode images of microscopic quality. The objective of this study was, by means of UBM use, to compare the anterior segment structures of the canine eyes, both with diabetic and non-diabetic cataract, in order to detect changes caused by DM. The parameters evaluated were: cornea thickness, anterior chamber’s depth, increased cellularity inside the anterior chamber, and iridocorneal angle measurement. Eighty-seven eyes of 47 dogs were examined, divided into three groups: control (GCO), non-diabetic cataract (GCAT) and diabetic cataract (GDM). The results showed that the diabetic group presented a higher cornea thickness than the other groups. The control group showed deeper anterior chambers without increased cellularity. When the iridocorneal angle measurements were analyzed, it was found that there were no statistically significant differences between the three groups. Based on these results, we can conclude that: the eyes of diabetic dogs with cataract showed a central cornea higher thickness compared to the eyes of dogs with cataract of different etiologies, and to dogs with normal eyes; there is a decrease of the anterior chamber depth and a increase of cellularity in the eyes of dogs with cataract compared to normal eyes, there is no significant difference between the iridocorneal angle measurement in the eyes of dogs with cataract, diabetic or not, and normal dogs.

E-mail: angsaf@usp.br Being the commonest ocular disorder, dense cataracts disable fundoscopic examination and the diagnosis of retinal disorders, which dogs may be predisposed. The aim of this study was to compare the electroretinographic responses recorded according to the International Society for Clinical Electrophysiology of Vision human protocol to evaluate retinal function of diabetic and non diabetic dogs, both presenting mature or hypermature cataracts. Full-field electroretinogram was recorded from 66 dogs, with ages varying from 6 to 15 years old allocated into two groups: (1) CG, non diabetic cataractous dogs, and (2) DG, diabetic cataractous dogs. Mean peak-to-peak amplitude (microvolts) and b-wave implicit time (milliseconds) were determined for each of the five standard full-field ERG responses (rod response, maximal response, oscillatory potentials, single-flash cone response and 30 Hz flicker). Comparing CG to DG, ERGs recorded from diabetic dogs presented lower amplitude and prolonged b-wave implicit time in all ERG responses. Prolonged b-wave implicit time was statistically significant (p< 0.05) at 30 Hz flicker (24.0 ms versus 22.4 ms). These data suggests full-field ERG is capable to record sensible alterations, such as flicker’s implicit time, being useful to investigate retinal dysfunction in diabetic dogs.

Numerous therapeutic methods involving both steroidal and non-steroidal anti-inflammatory agents have been used to reduce surgically-induced uveitis. The purpose of this study was to investigate the postoperative inflammatory reaction and intraocular pressure after phacoemulsification surgery for cataract extraction with and without intraocular piggyback lens (IOL) implantation in dogs. A total of 25 dogs were divided into two groups: Group 1 (with IOL implantation) and Group 2 (without IOL implantation). The performed surgical technique consisted of unilateral bimanual phaco-emulsification. Clinical assessment and intraocular pressure were measured before surgery (0) and at 3, 7, 14, 21, 28, 60 days after the surgery. Clinical assessment revealed inflammatory reaction more severe in dogs of G1 when compared to G2. Intraocular pressure did not differ significantly either between G1 and G2 or operated and non-operated eyes. Intraocular lens (IOL) implantation using piggyback technique with human IOL is a possible method to be performed in canine ophthalmology. However, nursing care is necessary as the the procedure induces inflammation and complications in the postoperative period.

#8 - A and B mode ultrasonography in preopera-tive evaluation of lens and posterior segment of dogs eyes with cataract, 30(2):121-126

Abstract in English:

ABSTRACT.- Martins B.C., Rodrigues Jr E.F., Souza A.L.G., Almeida D.E., Brito F.L.C., Canola J.C., Brooks D. & Laus J.L. 2010. A and B mode ultrasonography in preopera-tive evaluation of lens and posterior segment of dogs eyes with cataract. Pesquisa Veterinária Brasileira 30(2):121-126. Departamento de Clínica e Cirurgia Veterinária, Faculdade de Ciências Agrárias e Veterinárias, Universidade Estadual Paulista, Jaboticabal, SP 14884-900, Brazil. E-mail: jllaus@fcav.unesp.br Ultrasonography of the lens and posterior segment is an indispensable step in the preoperative evaluation of dogs with cataracts, since ophthalmoscopy is not feasible when there is opacification of the lens. This study evaluated the echographic conditions of cataractous lens and fundus of the eye in dogs affected by cataracts. The study was conducted in 30 dogs (56 eyes), 10 males and 20 females, with different types of cataracts at different stages of development. Echography in A and B modes, simultaneously, was carried out for the examination of the lens and posterior segment. The examinations revealed anterior cortical, posterior cortical and nuclear cataract in 12 eyes (21.4%), anterior cortical, posterior cortical, nuclear and posterior capsular in 23 eyes (41%), anterior cortical, posterior cortical and posterior capsular cataract in one eye (1.7%), anterior cortical and nuclear cataract in one eye (1.7%), anterior cortical, nuclear and posterior capsular cataract in five eyes (8.9%), and anterior cortical cataract in seven eyes (12.5%). Abnormal ultrasonographic alterations were observed in the posterior segment in 26 eyes evaluated (46.4%). Vitreal degeneration was detected in 12 eyes (21.4%), images of vitreal exudate or hemorrhage in seven eyes (12.5%), persistence of hyaloid artery in four eyes (7.1%) and lens subluxation in three eyes (5.3%). The results obtained reiterate the importance of ultrasonography in canine patients presented for cataract surgery given that alterations of the posterior segment are difficult to identify in a clinical examination when the lens is opacified.

A and B mode ultrasonography in preopera-tive evaluation of lens and posterior segment of dogs eyes with cataract.

Abstract in English:

ABSTRACT.- Safatle A.M.V., Hvenegaard A.P., Gomes D., Leandro D.C., Otsuki D. & Lisak R. 2010. [Full field electroretinogram in English Cocker Spaniels presenting cataracts.] E-mail: angsaf@usp.br Cataracts are one of the most frequent ocular disorders that affects dogs and leads to blindness. In the late stages, lens opacity unables fundoscopy and consequently the diagnosis of retinal degeneration such as progressive retinal atrophy (PRA), which contraindicates surgery. Aiming the return of vision, full field electroretinogram (full field ERG) becomes indispensable before surgery. As English Cocker Spaniels are predisposed to cataracts and retinal degeneration, the objective of this study is the evaluation of the full field ERG records performed in dogs. One hundred thirty-six full field ERGs were recorded from English Cocker Spaniels (62 males and 74 females, with ages ranging from 3 to 15 years old), between September, 2004 and May, 2009. Each examined dog presented cataracts and vision loss before the exam. Diagnosis of retinal degeneration was made based on ERG parameters which were measured from peak to peak amplitude and b-wave implicit time in the 3 different responses (rod response, maximal response and cone response). Amplitude measured in rod, maximal and cone response, as well as the implicit time recorded from dogs that did not present retinal degeneration were respectively: 71.55mV/65.15ms; 149.17mV/33.03ms; and 31.06mV/27.90ms. No response was recorded in 38 dogs. Low responses were recorded from the remaining dogs: 12.88mV/65.04ms, 24.16mV/36.25ms, and 8.36mV/31.38ms. Results showed that 122 animals (89.7%) presented progressive retinal atrophy. Therefore, we conclude that full field ERG must be performed in English Cocker Spaniels presenting cataracts before its removal due to high incidence of retinal degeneration in this breed.

It is necessary to identify very early any eye disease that could cause the failure of cataract surgery. The ocular ultrasonography or echography is a safe, non invasive, inexpensive and easy procedure that complements ocular examination. Two hundred and twenty five eyes of 123 dogs were evaluated using a 10 Mhz linear transducer ultrasound. The dogs were divided into three groups, as there is Group 1: 36 healthy control dogs; Group 2: 52 non diabetic dogs with cataracts; and Group 3: 35 diabetic dogs with cataracts; all of them were patients of the Veterinary Teaching Hospital, University of Sao Paulo. The ultrasonographic findings were in Group 1: vitreous degeneration in 43%, posterior vitreous detachment in 7.7%, asteroid hyalosis in 4.6% of the eyes; in Group 2: vitreous degeneration in 58.9%, posterior vitreous detachment in 8.4%, asteroid hyalosis in 2.1%, inflammatory or haemorrhagic process in 2.1%, retinal detachment in 4.3% of the eyes; and in Group 3: vitreous degeneration in 50.7%, posterior vitreous detachment in 13.8%, asteroid hyalosis in 12.3%, inflammatory or haemorrhagic process in 3.1%, retinal detachment in 3.1% of the eyes. It could be concluded that (1) diabetic dogs, mainly the ones with diabetes for more than 2 years, have greater chances to develop asteroid hyalosis; (2) there were no significant differences in retinal detachment frequencies for the diabetic and non diabetic dogs; and (3) the ocular ultrasonography is a safe, effective and important exam for the pre-operatory evaluation of dogs with cataracts.
