#11 - A multidisciplinary approach to diagnose naturally occurring bovine tuberculosis in Brazil, 33(1):15-20

Abstract in English:

ABSTRACT.- Marassi C.D., Medeiros L., Figueiredo E., Fonseca L.S., Duarte R., Paschoalin V., Oelemann W.M.R. & Lilenbaum W. 2103. A multidisciplinary approach to diagnose naturally occurring bovine tuberculosis in Brazil. Pesquisa Veterinária Brasileira 33(1):15-20. Laboratório de Bacteriologia Veterinária, Departamento de Microbiologia e Parasitologia, Universidade Federal Fluminense, Rua Prof. Hernani de Mello 101, Lab. 309, Niterói, RJ 24210-130, Brazil. E-mail: carladray@yahoo.com.br A herd infected naturally with tuberculosis was investigated by different diagnostic methods. Ninety days after a screening test that identified 21 cows as skin test positive, a Comparative Intradermal Tuberculin Test (CITT) was performed in those 21 cows and in 29 other randomly selected skin test negative cows. Milk samples and nasal swabs were collected prior to the CITT for bacteriological culture and PCR, while blood samples were collected for IFN release and antibody responses to MPB70 and MPB83, at three time points post tuberculin injection. Animals positive by CITT were slaughtered and disease confirmation undertaken. Based on the Kappa test, IFN was comparable to the standard tests (culture, PCR and CITT) at all three sampling points. Results from both antibody ELISAs were similar but were not comparable to the standard tests. T-test analysis of the CITT, IFN and ELISAs demonstrated that their performances were not correlated. There is increasing recognition that individually, available diagnostic tests do not detect all infected cattle. Therefore, a comprehensive strategy for the diagnosis of bovine TB should include test results for the detection of both cellular and humoral immune responses where there may be animals at different stages of infection.

RESUMO.- Marassi C.D., Medeiros L., Figueiredo E., Fonseca L.S., Duarte R., Paschoalin V., Oelemann W.M.R. & Lilenbaum W. 2103. A multidisciplinary approach to diagnose naturally occurring bovine tuberculosis in Brazil.

E-mail: luciano.lopes@embrapa.br This study aimed to evaluate the interference of tuberculin test on the gamma-interferon (INFg) assay, to estimate the sensitivity and specificity of the INFg assay in Brazilian conditions, and to simulate multiple testing using the comparative tuberculin test and the INFg assay. Three hundred-fifty cattle from two TB-free and two TB-infected herds were submitted to the comparative tuberculin test and the INFg assay. The comparative tuberculin test was performed using avian and bovine PPD. The INFg assay was performed by the Bovigam™ kit (CSL Veterinary, Australia), according to the manufacturer’s specifications. Sensitivity and specificity of the INFg assay were assessed by a Bayesian latent class model. These diagnostic parameters were also estimate for multiple testing. The results of INFg assay on D0 and D3 after the comparative tuberculin test were compared by the McNemar’s test and kappa statistics. Results of mean optical density from INFg assay on both days were similar. Sensitivity and specificity of the INFg assay showed results varying (95% confidence intervals) from 72 to 100% and 74 to 100% respectively. Sensitivity of parallel testing was over 97.5%, while specificity of serial testing was over 99.7%. The INFg assay proved to be a very useful diagnostic method.

#13 - Assessing the histopathology to depict the different stages of bovine tuberculosis infection in a naturally infected herd, 32(2):135-139

Abstract in English:

ABSTRACT.- Medeiros L., Marassi C.D., Figueiredo E.E.S., Leite J., Ferreira A.M.R. & Lilenbaum W. 2012. [ Assessing the histopathology to depict the different stages of bovine tuberculosis infection in a naturally infected herd]. Pesquisa Veterinária Brasileira 32(2):135-139. Laboratório de Bacteriologia Veterinária, Faculdade de Veterinária, Universidade Federal Fluminense, Rua Hernani Mello 101, sala 309, Niterói, RJ 24210-130, Brazil. E-mail: lusmedeiros@yahoo.com.br The standard method for detection of bovine tuberculosis (TB) is the single intradermal tuberculin test (SITT). Nevertheless, current studies suggest that a single test is not enough to detect all cattle infected by TB, particularly when animals present different stages of infection. A dairy herd comprised of 270 cows was studied and 15 were reactive to SITT plus nine inconclusive animals. Blood samples (for IFN and ELISA) were collected from these 24 cows. At 30 days after injection of PPD, all the cows that were reactive to any of the employed tests were slaughtered, and tissues were processed by Bacteriology, Histopathology (HP) and PCR. According to HP 33.4% of the animals were positive, 45.8% inconclusive and 20.8% were negative. The inconclusive samples came from IFN positive animals, signalizing recent infection. Regarding the animals that were negative to HP, all of them were identified by IFN while ELISA was negative. Immune responses are different in recent and advanced infections, what supports the identification between chronically or recently infected animals. This multidisciplinary approach is mandatory for the interpretation of the various tools that are frequently employed for the diagnosis of TB and mainly to identify all infected animals.

#14 - Bovine tuberculosis in the state of Paraíba: retrospective survey, 30(9):712-716

E-mail: sergio.azevedo@pq.cnpq.br The aim of this study was to determine the frequency of positive herds (foci) and positive animals for bovine tuberculosis in the state of Paraíba, Northeast region of Brazil. Data from the Agency of Agricultural Protection in the state, collected from its 23 microregions, during the January 2008 to July 2009 period, were used. During this period, 10,963 herds were examined and 54,472 cattle were submitted to the tuberculin test. For diagnosis the cervical and caudal-fold tuberculin tests were used as screening tests in dairy and beef cattle, respectively; as confirmatory test, comparative cervical test was used. A herd was considered focus when presented at least one positive animal. Of the herds investigated, 62 (0.57%) had at least one positive animal, and of the animals examined, 136 (0.25%) were positive. There was significant difference (p<0.001) in the proportion of positivity for females (0.32%) and males (0.04%). Despite low frequency of foci of brucellosis and seropositive animals, it is necessary to conduct measures including awareness of producers, surveillance in sanitary barriers and periodic surveys of epidemiological situation of this disease especially in the regions with highest frequency of infection, aiming to avoid, or at least minimize, the spread of the agent.

#15 - Comparison of the allergic diagnosis of bovine tuberculosis using the comparative and Stormont tests

Abstract in English:

The comparison of the comparative tuberculin test and the Stormont method, performed at the sarne groups of animals, gave highly convergent results in bovines free from tuberculosis and also in infected groups with strong reactors. In weak reactors the results were highly discordant. A group of 21 bovines with negative results at the comparative tuberculin test, but reacting to avian tuberculin, was negative with the Stormont test. Twenty six bovines which reacted strongly positive (4.0mm or more) at the comparative test, with or without non-specific reactions, showed also positive reactions in 88.4%, suspicious reactions in 7.7% and negative reactions in 3.9% at the Stormont test. A group of 19 bovines, initially tested by the Stormont method and considered as infected, when retested by the comparative test after 90 days, showed 84.2% positive reactors and 15.7% negative ones. Groups of bovines with suspicious (2.0 to 2.9mm) or weakly positive reactions (3.0 to 3.9 mm) at the comparative test, showed 20.0% positive, 20.0% suspicious and 60.0% negative reactors in the suspicious group and 53.3% positive, 13.3% suspicious and 33.3% negative reactions in the weakly positive reactors by the Stormont test. Another group of bovines gave suspicious reactions when initially tested by the Stormont method, but showed 50.0% positive, 9.3% suspicious and 40.6 negative reactions when retested by the comparative test. The discordant results of the present and other investigations do not allow to recommend the Stormont test for use in routine procedures for the diagnosis of bovine tuberculosis before a critical evaluation has been made, with slaughter and bacteriological exarnination of reacting animals.

#16 - Intermittent treatment of bovine tuberculosis with isoniazid

Abstract in English:

An intennittent treatment regímen for bovine tuberculosis with isoniazid was used. Fifty five positive and 13 suspect reactors were treated orally with cristalized isoniazid (25 mg/kg bw) mixed in a concentrated ration given 3 times a week, during 6 months. The cure of the tuberculosis was evaluated after the treatment by allergic desensitization with comparative tuberculinizations performed in 3 months intervals, during a year. The monitoring showed progressive reduction of the allergic sensitivity until definitive disappearance, At 90 days pos-treatment there were 31 negative, 19 suspect and 18 positive reactors; at 180 days, 47 were negative, 10 suspect and 11 positive; at 270 days, 51 were negative, 12 suspect and 5 weakly positive, and at 360 days, 63 were negative, 5 suspect and no one anymore positive. As it was demonstrated experimentally, the pos-treatment suspect reactors may be considered infection-free and so the cure rate was 100 percent. This experiment showed that the intennittent 3 week dosages were as good as the continuous regímen with the advantage of about 40 percent cost reduction.

#17 - Massal treatment of bovine tuberculosis with isoniazid

Abstract in English:

There were 409 bovines in a Swiss Brown-cattle herd in Mossoró, Rio Grande do Norte, Brazil, reacting positively to the tuberculin test. The tuberculosis was confirmed by necropsy, histopathological and bacteriological procedures. The infected animals were treated with purê crystalised isoniazid, administered orally at a dose of about 25 mg/kg/day, during 60 days consecutively and other 60 doses 3 times a week. The large number of reactors did not allow individual medication. Therefore treatment at a large scale was used in groups of animals varying in weight up to 100 kg. Isoniazid was mixed mechanically, daily, and applied at 12 g per 1 kg of high palatable concentrated ration, furnished in long double troughs. The quantity of feed was calculated based on the media of live weight and the numbers of animals in each group. The clinically affected animals showed recuperation starting with the second month of treatment. Thirty days after the end of the treatment the allergic monitoration began by comparative tuberculinizations at intervals of 2 to 3 months. The cure was evaluated by the gradual tuberculin desensitization. In most animals the tuberculin reactions became negative after 2 to 4 months post-treatment. At the end of this experiment 96.4% of the treated animals were cured. In slaughtered animals no lesions were found.

#18 - Nonspecific reactions in the diagnosis of bovine tuberculosis with PPD

Abstract in English:

The tuberculin test was performed on 3124 cattle from 24 tuberculosis-free herds from na area between the cities of Rio de Janeiro, São Paulo and Belo Horizonte, using bovine tuberculin PPD containing 5000 IU per dose and avian PPD containing 2500 IU per dose applied intradermically. An incre ase in the thickness of the skin fold equal to or greater than 3mm, independent of local clinical changes were considered positive. The results showed that 305 (9.7%) animals developed non-specific allergic reactions, whose prevalence ranged from 1.1 to 30.4% on all farms tested. The mean increase in the thickness of the skin folds was 4mm, while the mean of the strongest reaction from every farm was 5 .9mm. Tuberculin testing was also performed on 1945 cattle from 13 herds with tuberculosis, revealing 368 (18.9%) reactors and a prevalence range of 6.8 to 89.6%. The mean skin fold increase was 7.5mm, while the mean of the strongest reactions was 19.9mm. Furthermore, it was observed that 79% of the non-specific reactions showed skin fold increases of 34mm, which required the modification of the key for interpreting tuberculin reactions. Increases of up to 1.9mm were considered negative, between 2.0 and 3.9mm suspect, and 4.0mm and above positive, independent of local changes. Increases of 3mm or greater, if accompanied by pain, edema, exudate or local necrosis were considered positive. Simultaneous tuberculin testing is recommended in doubtful cases to rule out or confirm non-specific allergic reactions. When the modified key was applied to the positive reactors from the 24 tuberculosis-free herds, the number was reduced to 64 (2%).

#19 - Treatment of bovine tuberculosis with isoniazide

Abstract in English:

In a herd of 210 Holstein cattle, the intradermal tuberculin test revealed 149 positive reactors (70.9%). Tuberculosis was clinically evident in 21 animais (14.1%), the diagnosis being confirmed by both histology and the isolation of Mycobacterium bovis. The disease was treated with pure crystalized isoniazide, administered orally in doses of 25 mg/kg/day. The initial 60 doses were given on consecutive days and the remaining 60 doses three times a week on alternating days. During the medication, five clinically ill animals with advanced tuberculous lesions died, while the remaining sick animals recovered slowly. The efficacy of the therapeutic treatment was monitored for three years. Necropsy and bacteriological testing were done on 28 animais that died from other causes during the experimental period. Shrunken tuberculous lesions. Were found in eight of the animais necropsied, surrounded and invaded by an intense reaction of fibrous connective tissue. Direct culture and inoculation of suspect material into guinea pigs proved negative in all cases. Tuberculin testing, begun one month after the termination of the medication and continued at intervals of two, three and fout month periods, permitted the accompaniment of the allergic desensitization over a period of 28 months. The mean o f the increased thickness of the skin fold at the site of tuberculin inoculation of 149 animais was 7.5 mm before treatment with isoniazide and gradually fell to 1.4 mm by the end of the observations. At the termination of the experiment, only five animals showed reactions above 3 mm. Considering these animais and the five that died during medication as incured, the therapeutic efficacy of the treatment with isoniazide was 93.28%.
