#1 - Acetabular ventroversion using the sacroiliac wedge, with or without pelvic osteotomies in dogs: an ex vivo study

Abstract in English:

Canine hip dysplasia (CHD) is a common condition observed in the surgical clinics for small animals. Among the surgical techniques for management of CHD, triple pelvic osteotomy and sacroiliac wedge promote acetabular lateral axial rotation (ventroversion), increasing acetabular coverage and joint stability. The present study aimed to evaluate radiographically, by measuring the Norberg angle (NA) and the acetabular coverage percentage (ACP), the acetabular ventroversion induced by the sacroiliac wedge technique, with or without pelvic osteotomies; we also checked the feasibility of wedges made of polyamide with an angulation of 20° and 30°. The software used to measure NA and ACP was AutoCAD® 2009. Pelves from 10 canine corpses were evaluated radiographically at four time-points: M0 (Control Group), M1 (wedges of 20° and 30°), M2 and M3 (wedges associated with bilateral pubis and ischium osteotomies, respectively). There was no significant increase in the acetabular ventroversion at M1, M2, and M3. The polyamide sacroiliac wedge technique proved to be feasible, stable, and easy to apply. Further, the software proved to be efficient and easy to use for NA and ACP measurements. In the present study, even in the cases of non-dysplasic adult canine corpses, it was concluded that the sacroiliac wedge technique does not require to be accompanied by pubis and ischial osteotomies because they did not significantly increase the NA and ACP.

A displasia coxofemoral (DCF) é afecção comum na clínica cirúrgica de pequenos animais. Entre as técnicas cirúrgicas para controle da DCF, a osteotomia pélvica tripla (OPT) e a cunha sacroilíaca (CSI), promovem rotação lateral acetabular no eixo axial (ventroversão), aumentando a cobertura acetabular e a estabilidade da articulação. Desta forma, o presente estudo objetivou avaliar radiograficamente, por meio da aferição do ângulo de Norberg (NA) e da porcentagem de cobertura acetabular (PCA), a ventroversão acetabular induzida pela técnica da cunha sacroilíaca, associada ou não às osteotomias pélvicas, além de verificar a exequibilidade das cunhas confeccionadas de poliamida com angulação de 20ο e 30ο. O software utilizado para aferir o AN e o PCA foi o AutoCAD® 2009. Dez pelves de cadáveres caninos foram avaliadas radiograficamente em quatro momentos: MO (Grupo Controle), M1 (cunhas de 20ο e 30ο), M2 e M3 (cunhas associadas à osteotomia bilateral do púbis e ísquio, respectivamente). Não houve aumento significativo da ventroversão em M1, M2 e M3. A técnica de cunha sacroilíaca de poliamida mostrou-se exequível, estável e de fácil aplicação. Não obstante, o software utilizado mostrou-se eficiente e de fácil utilização nas aferições do AN e PCA. Neste estudo, mesmo tratando-se de cadáveres de cães adultos e de maioria não displásicos, concluiu-se que a utilização da técnica de cunha sacroilíaca não necessita de associação à ostectomia púbica e a osteotomia do ísquio por não promoverem aumento significativo do AN e da PCA.

E-mail: anemiaveterinaria@yahoo.com.br Although the prevalence of feline infectious peritonitis (FIP) is high worldwide, recent anatomopathological studies about this disease are scarce. Information on the microscopic characteristics of the bone marrow in FIP-affected cats are absent in the available literature. Based on this, the purpose of this article is to describe bone marrow lesions seen in spontaneous cases of FIP. The bone marrow collected systematically from the femoral diaphysis of 16 cats necropsied in the Laboratory of Veterinary Pathology (LPV) of the Federal University (UFSM) of Southern Brazil, between January 2000 and June 2017, with a definitive diagnosis of FIP, were evaluated phenotypically (histopathology - hematoxylin and eosin and histochemistry - Perls stain) and immunophenotypically (immunohistochemistry using myeloid - Anti‑MAC387, and lymphocytic - Anti-CD79 αcy and Anti-CD3 markers). Regardless the following was observed the clinicopathological form of the disease (“dry” - noneffusive or “wet” - effusive): 1) myeloid hyperplasia; 2) erythroid hipoplasia; 3) megakaryocytic dysplasia (dismegakaryocytopoiesis); and 4) medullary plasmacytosis. Exclusively in cases of “dry FIP” was bone marrow and hepatic hemosiderosis. These lesions allowed establishing that cats with FIP develop myelodysplasia, a myeloproliferative lesion very similar to that reported in HIV-infected humans. It is suggested that, based on the findings described here, myelodysplasia is considered to be the main cause of hematological abnormalities observed in FIP, especially for non-regenerative anemia and thrombocytopenia, frequently developed by patients.

Não obstante, as características microscópicas da medula óssea de gatos com PIF estão ausentes da literatura consultada. O objetivo deste artigo é descrever alterações medulares ósseas vistas em casos espontâneos de PIF. As medulas ósseas colhidas sistematicamente da região diafisária dos fêmures de 16 gatos necropsiados no Laboratório de Patologia Veterinária (LPV) da Universidade Federal de Santa Maria (UFSM), Rio Grande do Sul, entre janeiro de 2000 e junho de 2017, e que tiveram diagnóstico definitivo de PIF, foram avaliadas fenotípica (histopatologia - hematoxilina e eosina e histoquímica - reação de Perls) e imunofenotipicamente (utilizando marcadores mieloides (anti-MAC387) e de linfócitos (anti-CD79αcy e anti-CD3). Os resultados permitem afirmar que, independentemente da apresentação clinicopatológica da doença ocorrem as seguintes alterações: 1) hiperplasia mieloide; 2) hipoplasia eritroide, 3) displasia megacariocítica (dismegacariocitopoiese) e 4) plasmocitose medular. Exclusivamente nos casos de PIF seca há hemossiderose medular óssea e hepática. Essas alterações permitem estabelecer que gatos com PIF desenvolvem mielodisplasia, uma lesão mieloproliferativa muito semelhante àquela relatada em humanos infectados pelo HIV. Sugere-se que a partir dos achados aqui descritos, a mielodisplasia seja considerada a principal responsável pelas alterações hematológicas observadas na PIF, especialmente pela anemia e trombocitopenia arregenerativas frequentemente desenvolvidas pelos pacientes com essa doença.

E-mail: malimistieri@gmail.com Considering the advantages of the interlocking intramedullary nail (IN) in relation to bone plates in the femoral stabilization, this study aimed to evaluate the feasibility and effectiveness of IN application post intertrochanteric varus osteotomy (IVO). For this purpose, 10 canines cadavers were used. On the left femurs, the fixation was obtained with IN (IN group) and rights fixed with dynamic compression plates and screws (plate group). Was compared the angles of Norberg and inclination of the head and femoral neck (IHF) before (T0) and after (T1) IVO. The results of time spent for placement of implants and biomechanical resistance to compression and torsion were also confronted between groups. There was an increase of the Norberg angle between T0 (106.84±5.55o) and T1 (111.22o±3.89), only in IN group (p<0.05). However, reduction of IHF angle after IVO (T1) was observed in both, the plate group (T0=127.6±4.70o e T1=110.06±10.61o, p<0.05) and IN group (T0=126.43±5.87o e T1=116.87± 8.62 o, p<0.05). The placement times of the implants did not differ statistically and only the compression biomechanical test revealed differences between groups, with greater resistance (P=0.033) of the plate group (863.3±74.46N/mm) compared to IN group (586.7±44.10N/mm). Thus, stabilization through IN was feasible and effective. Although the compression biomechanical results has demonstrated a higher fragility of IN in relation to the compression plate, their values are above the forces, reported in literature, acting in normal dog gait.

E-mail: malimistieri@gmail.com Tendo em vista as vantagens das hastes intramedulares bloqueadas (HIB) em relação às placas ósseas na estabilização femoral, o presente estudo objetivou avaliar a exequibilidade e eficácia da aplicação da HIB pós osteotomia intertrocantérica varizante (OIV). Submeteu-se 10 cadáveres caninos à OIV. Nos antímeros esquerdos obteve-se a estabilização por meio de HIB (grupo HIB) e, nos direitos, com placas de compressão dinâmica (grupo placa). Foram comparados os ângulos de Norberg e de inclinação da cabeça e colo femoral (ICF) antes (T0) e após (T1) a OIV. O tempo dispendido para a colocação dos implantes e resultados de resistência biomecânica à compressão e torção também foram confrontados entre os grupos. Houve aumento do ângulo de Norberg entre T0 (106,84±5,55o) e T1 (111,22o±3,89), apenas no grupo HIB (p<0,05). No entanto, redução do ângulo de ICF após OIV (T1) foi observada tanto no grupo placa (T0=127,6±4,70o e T1=110,06±10,61o, p<0,05) quanto no grupo HIB (T0=126,43±5,87o e T1=116,87±8,62o, p<0,05). Os tempos de colocação dos implantes não diferiram estatisticamente e apenas o teste biomecânico de compressão revelou diferença entre os grupos, com maior resistência (P=0,033) do grupo placa (863,3±74,46N/mm) em relação ao grupo HIB (586,7±44,10N/mm). Deste modo, a estabilização por meio de HIB foi factível e eficaz. Embora o resultado biomecânico de compressão tenha demonstrado maior fragilidade da HIB em relação à placa de compressão, seus valores estão acima das forças atuantes, reportadas na literatura, na deambulação normal de cão.

[Pathological findings in dogs with renal dysplasia in Southern Brazil.] Achados patológicos em caninos com displasia renal no Sul do Brasil. Pesquisa Veterinária Brasileira 34(12):1227-1230. E-mail: davetpat@ufrgs.br Renal dysplasia results from a disturbance which occurs during nephrogenesis. The disease may be unilateral or bilateral, and leads to abnormal kidney differentiation leading to renal failure in young dogs. Several agents, including viruses, can cause the dysplasia till three months after birth, when the early nephron formation of the developing kidney is completed. Eleven renal dysplasia cases were diagnosed in 186 dogs with renal failure of 5,846 dogs necropsied in the Veterinary Pathology Sector, Federal University of Rio Grande do Sul, in the period 2002-2013. Tissue samples collected during necropsy were fixed in 10% formaldehyde and stained with hematoxylin-eosin (HE) and Masson’s trichrome protocol. Upon necropsy, kidneys were pale, smaller than normal, irregular and firm, and presented lower cortex diameter. Some exhibited cysts and whitish, parallel radially arranged striae in the medulla. Primary and secondary renal lesions were observed during histological examination. Primary lesions included fetal glomeruli and tubules, quantitative reduction in glomeruli, adenomatous tubules, and persistence of metanephric ducts. As for secondary lesions, Masson trichrome staining revealed intense interstitial fibrosis in all cases; furthermore, dilation of tubules and of Bowman’s capsules, glomerular atrophy, and glomerulosclerosis were observed. Even though most cases were associated with breed, the results obtained indicate high prevalence of renal dysplasia in mixed-breed dogs, possibly because they were the majority of animals referred for necropsy. Although primary lesions were easily identified using HE, Masson’s trichrome protocol is useful to characterize the extent of fibrosis.

E-mail: davetpat@ufrgs.br A displasia renal resulta de um distúrbio na nefrogênese, com diferenciação anormal dos rins, podendo ser unilateral ou bilateral, e causa insuficiência renal em cães jovens. Qualquer agente agressor, incluindo vírus, pode causar a lesão até três meses após o nascimento, quando encerra o desenvolvimento embrionário do néfron. Foram diagnosticados 11 casos de displasia renal dentre 186 casos de cães com insuficiência renal de um total de 5.846 cães submetidos à necropsia no Setor de Patologia Veterinária da Universidade Federal do Rio Grande do Sul no período 2002-2013. Amostras teciduais coletadas na necropsia foram fixadas em 10% de formol e coradas com hematoxilina e eosina (HE) e tricrômico de Masson. Na necropsia, os rins estavam pálidos, diminuídos de tamanho, irregulares e firmes, com diâmetro da cortical diminuído. Alguns ainda eram císticos e apresentavam estriações brancacentas paralelas dispostas radialmente na medular. Ao exame histológico os rins exibiam lesões primárias e secundárias. As lesões primárias identificadas foram glomérulos e túbulos fetais, diminuição quantitativa de glomérulos, túbulos adenomatosos e persistência de ductos metanéfricos. Dentre as lesões secundárias, a coloração de tricrômico de Masson evidenciou intensa fibrose intersticial em todos os casos. Além disso, foram observadas dilatação de túbulos e da cápsula de Bowman, atrofia glomerular e glomeruloesclerose. Ainda que a maioria dos casos tenha sido relacionada com linhagens raciais, esse trabalho demonstra uma alta frequência de diagnóstico de displasia renal em cães sem raça definida, possivelmente devido ao fato de que estes representam a maioria dos cães encaminhados ao laboratório. Apesar das lesões primárias serem facilmente identificadas pela coloração de hematoxilina eosina, a coloração de tricrômico de Masson auxilia na identificação da extensão da fibrose.

[Laparoscopic nephrectomy in Pampas fox (Pseudalopex gymnocercus) with renal dysplasia.] Nefrectomia videolaparoscópica em Graxaim-do-campo (Pseudalopex gymnocercus) com displasia renal. Pesquisa Veterinária Brasileira 34(8):776-779. E-mail: mauriciovelosobrun@hotmail.com Pampas fox (Pseudalopex gymnocercus) inhabits east-central South America, from southern Brazil to eastern Bolivia. The renal dysplasia comprises a spectrum of abnormalities and is often reported in dogs of various breeds; but this disease reports involve mostly necropsy findings. Congenital renal diseases in dogs have a progressive character, so that treatment is limited and directed to retard the disease. This report describes a laparoscopic nephrectomy in pampas fox with renal dysplasia. Right kidney disease was suspected by ultrasound and intravenous urography. For the surgical procedure we used three portal accesses (10, 10, 5mm), rigid endoscope 10mm/00 and clips for hemostasis of the renal vessels. Due the absence of reports of laparoscopic nephrectomy in pampas fox and scarcity of surgical techniques for this species, the operative technique used was similar to that used in the domestic dog. The pampas fox was monitored for 30 days after surgery without the occurrence of complications. This report demonstrates that the proposed technique is appropriate to perform laparoscopic nephrectomy in pampas fox.

A displasia renal compreende um espectro de anomalias e é frequentemente relatada em cães de diversas raças, sendo a maioria dos relatos achados de necropsia. A maior parte das doenças renais congênitas nos cães apresenta caráter progressivo, portanto o tratamento é limitado e direcionado a fim de retardar a progressão da doença renal estabelecida. No presente relato é descrita a realização de nefrectomia laparoscópica em graxaim-do-campo (Pseudalopex gymnocercus) com displasia renal. Suspeitou-se de doença no rim direito por meio de ultrassonografia e urografia excretora. Para o procedimento cirúrgico foram utilizados três portais (10, 10, 5mm), endoscópio rígido de 10mm/00 e clipes para hemostasia dos vasos renais. A técnica cirúrgica utilizada neste canídeo selvagem foi semelhante à usada em cão doméstico, devido à escassez de relatos em cirurgias em graxaim e a inexistência de descrições de nefrectomia videolaparoscópica em graxaim-do-campo. O animal foi acompanhado por um período de 30 dias pós-cirurgia sem a ocorrência de complicações. O presente relato demonstra que a técnica proposta foi adequada para a realização de nefrectomia videolaparoscópica em Graxaim-do-campo.


#6 - Chondrodysplasia in cattle in Southern Brazil, 33(10):1195-1200

[Chondrodysplasia in cattle in Southern Brazil.] Condrodisplasia em bovinos no Sul do Rio Grande do Sul. Pesquisa Veterinária Brasileira 33(10):1195-1200. Laboratório Regional de Diagnóstico, Faculdade de Veterinária, Universidade Federal de Pelotas, Campus Universitário s/n, Pelotas, RS 96010-900, Brazil. E-mail: alschild@terra.com.br Fourteen cases of chondrodysplasia in cattle are described. Epidemiological and clinical data were obtained from filed necropsy protocols. A histological study of the lesions was performed on long and skull base bones. Eleven cases of chondrodysplasia Telemark type and three Bulldog (Dexter) type were observed. Thirteen out of 14 cases occurred in Jersey cattle and one in Shorthorn. It was concluded that the gene carrier of chondrodysplasia is present in the Jersey population of the region, and breeding measures such as the use of bulls from other regions and/or progeny testing or identification of undesirable genes using molecular techniques should help reduce the occurrence of these cases in the Jersey population of the region.

[Chondrodysplasia in cattle in Southern Brazil.] Condrodisplasia em bovinos no Sul do Rio Grande do Sul. Pesquisa Veterinária Brasileira 33(10):1195-1200. Laboratório Regional de Diagnóstico, Faculdade de Veterinária, Universidade Federal de Pelotas, Campus Universitário s/n, Pelotas, RS 96010-900, Brazil. E-mail: alschild@terra.com.br Descrevem-se 14 casos de condrodisplasia em bovinos. Os dados epidemiológicos e clínicos foram obtidos de protocolos de necropsia e o estudo histológico das lesões foi realizado em fragmentos de ossos longos e ossos da base do crânio dos 14 casos estudados. Onze casos eram de condrodisplasia tipo Telemark e três, tipo bulldog (Dexter). Treze dos 14 bovinos afetados eram da raça Jersey e um era da raça Shorthorn. Concluiu-se que o gene transmissor das condrodisplasias encontra-se presente na população Jersey da região e medidas, como utilização de reprodutores de outras regiões e/ou com teste de progênie ou identificação de genes indesejáveis por meio de técnicas moleculares, devem contribuir para diminuir a ocorrência destes casos na população Jersey da região.

The goal of this study was to apply the techniques of sacroiliac wedge and TPO to canine cadavers and radiographically evaluate the efficacy of acetabular ventroversion. Ten cadavers of adult dogs weighting 15-25 kg were used. To each right hemipelvis the technique of TPO with 20° and 40° plates was applied. In the left hemipelvis, 20° and 40º wedges in the sacroiliac joint were applied. Radiographic evaluations in standard projections were carried out for measurement of the acetabular covering of the head of the femur in the two techniques. There was no significant differences between TPO and the use of sacroiliac wedge using implants of 20° and 40°, however a significant difference (p<0,05) could be observed before and after application of the 20° and 40° implants, as well as between those that received 20° and of 40° implants. The application of sacroiliac wedge produced similar results of those observed in TPO, and it is more feasible.

O objetivo deste estudo foi aplicar as técnicas de cunha sacroilíaca e OPT em cadáveres e avaliar radiograficamente a eficácia da ventroversão acetabular. Para tal, foram utilizados 10 cadáveres de cães, adultos, com 15-25 kg. Em cada hemipelve direita foi realizada OPT com placas de 20° e 40°. Na hemipelve esquerda foram aplicadas cunhas nas articulações sacroilíacas de 20° e 40°. Avaliações radiográficas em projeções ventrodorsais foram realizadas para mensuração da cobertura acetabular à cabeça femoral nas duas técnicas. De acordo com os dados obtidos pode-se observar que não houve diferença entre a técnica de OPT e o uso de cunha sacroilíaca utilizando implantes de 20° e 40°, mas ocorreu diferença significativa (p<0,05) entre os cães antes e após a aplicação dos implantes de 20° e 40°, e também entre os que receberam implantes de 20° e os de 40°. A aplicação de cunha sacroilíaca produziu resultados semelhantes à OPT, e também se mostrou como de mais fácil aplicação.

In a second surgical intervention on the same dogs the acetabulum was approached and deepened, and the femoral head ligament and the joint capsule were reconstructed. In general, within 30 days of the surgery dogs could stand on the operated member to walk. Except for two dogs, all the others recovered pelvic member locomotive ability within 60-90 days after surgery. It is concluded that acetabuloplasty is a good alternative for treatment of severe canine hip dysplasia.

Dorsal acetabular denervation is a feasible surgical technique in treatment of pain secondary to hip dysplasia in dogs, with significant decrease of pain after 2 days of treatment, improvement of quality of life, decrease of lameness, and consequently joyful animals and owners extremely satisfied with the results obtained by the proposed treatment. The surgical technique must include the curettage of nerve fibers from the cranial-dorsal and dorsal region of the acetabular periosteum.

After dorsal opening of the middle gluteal muscle, the insertion of the deep gluteal muscle is inserted for the access to the mentioned regions. With the help of a curette, the periosteum of the cranial and dorso-lateral acetabular margin was removed until the bone cortex was exposed, in all patients. In all animals, the approach was possible, both in the cranial as well as in the caudal faces for the access and curettage of the acetabulum, with the technique described, with a skin incision of about 3.2cm. The curettage of the cranial portion of the acetabulum was, in all cases, realized without the visualization of the bone, but of easy achievement and without intercurrences of nervous or muscular injuries. In all the approaches, it was possible to visualize the articular capsule of the coxo-femoral joint, and in none of the animals, this structure was incised for a technique mistake. The surgical approach described in the present study is feasible for this kind of surgical procedure in dogs, promoting adequate access, with minimal invasion and with no complications of any nature.
